Frequently Asked Questions

Does a Category I code guarantee coverage?

No. A code identifies a service. Coverage and payment depend on the applicable policy, patient benefits, medical necessity, documentation and billing requirements. FDA clearance alone does not establish payer coverage. [1, 6]

Can both FFRCT and plaque analysis be billed from one CCTA examination?
Both analyses may be considered when separately ordered or otherwise ordered in accordance with payer rules, medically necessary and performed. Verify coverage for each service and review current coding instructions and payer edits before reporting them together. Do not assume both services are payable for every CCTA examination.
What do global, TC and 26 mean?
Global payment covers both technical and professional services. Modifier TC identifies a separately billed technical component; modifier 26 identifies a separately billed professional interpretation and report. Providers should bill only the components they are entitled to report. [2, 4]
Are these the amounts every provider will receive?
No. These are national Medicare reference amounts. Actual allowed amounts and payments may differ because of geographic adjustments, QP status, facility-specific adjustments, payer contracts and other applicable policies. The figures do not represent guaranteed collections or patient out-of-pocket costs. [1, 2, 4]
